13 February 2007: Clinical Solutions is set to launch its new system for the management of Long Term Conditions (LTCs) at Healthcare Computing 2007 in March. The system will transform the way patients with long term conditions are provided for by healthcare services in Europe.
The Long Term Conditions Management (LTCM) programme enables GPs and nurses to more easily support care of patients with long term conditions. By placing patients’ needs at the centre of the service, the case management and decision support software, integrated with telemonitoring devices, allows clinicians to support and advise patients on ongoing care without the need for a home visit or surgery appointment.
The benefit of the system to healthcare professionals is that it will enable them to allocate a personalised level of care to patients with LTCs, and minimise any unnecessary hospital bed days or appointments.
Jackie Rice-Haga, Director of Nursing from Clinical Solutions explains, “We must acknowledge the fact that there is an ageing population around the world that will require ongoing care. Within health services globally, there is an ever increasing need to use existing resources in the most appropriate way. This system utilises primary care givers time more effectively to empower patients with long terms conditions to take more responsibility on how they look after themselves.
Visitors to the stand will also be able to see demonstrations of the new Bio Surveillance system. The web-based system can quickly triage and advise people who may have been exposed to epidemiological infection or emergency medical situations.
