In the UK, there are an estimated 17.5 million people with one or more long term conditions. These patients account for 80 per cent of all GP consultations and 60 per cent of hospital bed stays. As the population ages, healthcare service providers are going to see an increase in these figures. With these statics in mind there has been a great deal of emphasis over recent years in developing devices for the home to monitor patients alleviating unnecessary GP and hospital visits. Whilst this is a positive step forward there has been limited focus on developing decision support solutions to pro-actively help patients and healthcare professionals in managing poor lifestyle choices that lead to hospitalisation.

In response, Clinical Solutions is exploring a Proof of Concept (POC) patient outreach solution utilising our leading decision support applications and clinical content. IntefleCS® Long Term Conditions Management POC will allow customers to manage patient programmes from enrolment and decision support based care planning through to graduation..

The IntefleCS® Long Term Conditions Management POC will combine our clinical expertise with web technology, telephone communications, and tailored software. The concept is that it can give patients autonomy over their care by educating and supporting them through a tailored programme designed to help them manage their specific condition. With the input of the clinician, patients can help to structure their own care plan, increasing their chances of success. Clinicians provide reassurance and reinforcement through regular contact. By linking with telehealth or telecare monitors, the clinician can receive accurate feedback and monitor progress alongside the patient.
